    You have two forms:

    1) http://www.jotformz.com/form/72696858784683 - Cotação do Seguro de Automóvel 

    2) http://www.jotformz.com/form/81264538884670 - Emissão de Proposta

     

    They have been found to be collecting CPF numbers, CNH numbers, and RG numbers. According to our terms of use this is forbidden.

     

    If you wish to keep these fields on your forms, then you must add:

    1) Your companys terms of use. You can use any of these widgets:

    OK, as agreed I have reactivated your account. Please make the adjustments now, and notify us when you are done, so that we can white list your forms. When we white list them the system will not autosuspend you again.

    Yes, you should do this as soon as possible. Because the system will autosuspend you again if you don't fix it.
