Text field not displaying on PDF submission

    I have a form titled "Cancellation Policy" which has some text I've put outlining our policy, then the patient dates and signs at the bottom - the issue is once they've signed it and we download that as a PDF, the text regarding the policy is not there, only the date and name.  

    We want to upload that PDF to our patient system, but without that policy text, it's kind of useless...

    Hope there's a better option here as many of my forms are like this, our policy then the patient signing off on that.

    You need to enable the "Show Headers and Texts" option in the Submission Panel to show the texts in the PDF.
