Display a block of text on the outputted form

    Hi guys, great tool.

    I'm setting up a patient sign up sheet for a client. He requires his patients to put in their data in, and then at the bottom there is a consent section which needs to be displayed both to the person who "Agrees" and for when the data is passed onto other people to see (on the output). As you can see I've tried a couple of ways but it just won't show up on the output page for us to print.

    It's this bit at the bottom:

    "Your consent is required for this practice to disclose and receive information involved in your health care management; these include treating doctors and specialists outside this practice, Workcover or third party insurance companies, any medical tests or reports that are relevant to your ongoing treatment. In signing below you are consenting and also agreeing that any accounts given to you on behalf of the doctors are the responsibility of you the patient. By giving us your insurance details, you are allowing us to claim any professional services rendered on behalf of the doctor, directly from your health fund."

    In addition to displaying that text we also want all fields to display in the output whether they've been filled or not. Is that possible?

    Hi

    What do you mean by 'outputted form', are you generating a PDF of the submissions?

    If so, you should be able to enable the rendering of those fields by going into the submissions area of the form, select 'properties' by clicking on the grey cog on the right, and then

    deselecting the 'Auto Hide Empty Fields' option

    and selecting the 'Show Headers and Texts' column.

    Can you try that out? Let us know if you cant find that properties panel.
