Square Payment Integration: getting card_nonce or customer_card_id must be set error

I have linked by "2018 Banquet RSVP" form to square and each time I try to submit a credit card payment I get a Payment error message. A similar form I created on Monday works fine. What is wrong with this form?

It appears to be well setup, please try disconnecting and connecting to your Square payment integration again, if the issue continues, please contact Square support to see if there is something there missing or not setup.
