How to create fillable PDF form?

I would like to use my own PDF onto which I can have my patients check boxes and sign with a stylus. Does JotForm offer this? Or must I use a template or form created in JotForm to collect the data? I would prefer the cleanliness of using my existing forms and simply get signatures on them electronically and automatically saved without having to use printed paper and a scanner like we do now. Thanks.
