This is a link we use that used to work and is no longer working. It appears to be because of problems with blank spaces (%20) causing an error. I've manufactured a few links to show the issue. Anytime there is a blank space the link doesn't work.
These don't work:

It is a standard in URL formatting to replace spaces with %20 and is accepted in ALL browsers. In fact, browsers explicitly replace the spaces since URLs cannot contain such.
I've reviewed all 4 URLs you have mentioned and ALL 4 of them are not working. Please note that when using prepopulate method, use a question mark (?) right after the form URL before starting any field=value paring.

Thank you for the point about the "?" - we're trying taht.
However - if you copy the two working links I put in my note above and paste them into the url bar, they work. But if you click on them they don't. Please try this.
I've had that same issue with using the links with the ? in them before the first field-value pairing - they sometimes don't work when clicked from within a sent email.

All form URLs doesn't work for me. It shows 404 page.
Please follow my colleague's suggestion, it should work for all the forms on any browser since that's the proper syntax order for query string.
Make sure the hyperlinks on email points to itself and has the same syntax.
So like my colleague, it should be:

If you are using fields as the value, you might want to use URLENCODE parameter.
