For some reason when my customers fill out the form on the url below
I'm not getting the data from all the fields.
The results in getting has
But as you can see within jotform, my form is completely different
I hope Im explaining correctly

You have modified Notification Email template body, but our system does not update modified templates. Due to that change, your Notification Email body is not up to date.
In order to solve the problem, please delete your current Email Notification, and then add a fresh one.
Please give it a try and let us know if you need any further assistance.
