Need code to embed the form on new web site

I just need the code to imbeded the jot form "fill out form" for our new web site for the request a coupon forms we have for both the mobile and desktop codes i believe?

It is on our web site at the $69 new patient deal to request a coupon i want your form their instead of theirs so the coupon will get sent out.
