How to collect multiple individual data from users

    Is there a way that a user could use this form for multiple memberships but submit the form only one time and pay for multiple membership at one time?

    For example, the price for 1-3 members is $75.  Could the user submit a form for Joe who is a judge and another for Jane who is a clerk.  The address, phone number and email address may be different for each.  The user would use one credit card to pay a total of $75 for both memberships?

    I know I add a choice of products (1-3 members for $75 or 4-10 members for $100 or 11 - 15 members for $200).  But I don't how to allow for multiple names, titles, addresses for each individual membership on one form.

    If you want to collect multiple information on your form with the same set of questions dynamically (just when it is needed), then I suggest that you use a Configurable List widget. The widget allows your users to add multiple entries with different information to fields of different types.
