Do you have any other recommendations or documentaiton to create a product order form?

Thank you so much for your expedient reply to my question about an affiliate form and order form. I think I understand what to do and how to create.

I have played around with several templates. I think the closest I have found to what I am trying to do is the Wine Product Order Form, however, if you have any other recommendations or documentation, I will really appreciate any guidance.

Thank you for your comments. I have checked your account and see that there are two forms in it. However, it looks like both the forms are not having any payment method integrated. In order to add the products/services that you want to sell through JotForm, you may add a payment field to the form and configure them by following the guide below:
