Page form doesn't display at all.

It seems to be a lightbox issue with your forms.

Your form is working fine in my end upon checking. The issue has something to do with the SSL limit which is why your form is not working on your website. Free account are only allowed up to 10 SSL secure submissions per month. I would suggest you to upgrade your account to Premium to increase submission quota. However, if you don't want to upgrade your account then please use a non-secured form into your website to avoid this from happening again. I will now re-activate your account for the main time in order for you to update your form code that you embed into your website or upgrade your account.

The form isn't setup for SSL.
Your lightbox test doesn't work even. Something is wrong.

Your lightbox seems to work for me as well. I even manage to fill out the form and submit through your website. Let me know if you receive the submission, please.
Please see this screenshot as well. I have also tried to copy/paste the lightbox code you've provided and it's good. Not sure what's happening your end.
Have you tried using a different browser? Have tried using Firefox and Chrome looks fine. Clearing cache might help as well.
