Some fields are not included on the email confirmation.

    The confirmation email for https://form.jotform.com/70947148938168 looks like this when I'm creating it:

    but when the email is sent it looks like this:

    The email was copy and pasted from another form, so I suspect that is the problem, but everything looks fine when I'm editing it, so I'm not sure how to fix it.

    Your email notification is setup correctly. The reason you are seeing those boxes (Name of Commercial Interest, Who has the relationship etc..) are because of the configurable list widget. That is how the widget is rendered in the email body.

    The issue isn't the Configurable Widget field. The text in the row between the form title and "Role" is missing. Another issue is the fact that the fonts in the email (specifically the "Role" and "input7" don't match what is in the editor, but I'm less concerned with that than with the row of text that is not showing up when the email is sent.

    Your autoresponder emails are conditionally sent, one of the autoresponders doesn't have any text in the body of the email:

    Check to see if that is the one that is being received instead of the one containing your text.

    In my original message, the first screen shot is the email as it appears in Jotform. The second screenshot is the email I received when I filled out the form in a way that would generate that email.

    I understand that the emails are conditionally sent, that is why the {form_title} header on the the email without any text is a different color, so it's obvious that it's not the same email.

    I solved this on my own by adding a new row that shows up when the email is sent, but it is frustrating that I still don't know why it is happening, so there's no way to prevent the issue in the future.

    If the formatting was copied directly from the text of another email, it could have been inserted differently in the code of the new notification.  Copying the full source code instead of just the text of the notification would ensure that the exact same code is used and inserted in the same manner:
