How to put question on a line by itself, and the list of choices in 3 columns below it without overlapping?

I've asked something similar, but it was putting two different questions side by side.

This is one question with a fairly long list of check box answers.  I'd like to put the question on a line by itself, and the choices in maybe 3 columns below it.  I should have plenty of width, but when I tried, the answers were overlapping.

If you have a FAQ and examples of this, would appreciate knowing how to find it in the future, also.

I put a link to my form below... you'll notice it is the "Primary Reasons" question, with the one long column of answers.

Is this what you are looking for ?

Please change the label align to TOP, and then applying 3 columns to the boxes

We try to keep adding new guides to our User Guide section, so I encourage you to check on that section every now and then

Yes, more or less… I got them to that stage once… but I just didn’t like how they wrapped, when there was enough width for them not to…

Christine, we may also try to play with the .form-checkbox-item CSS class, but we will need some more information from you on how to style the options.

    Thank you.
