If I have a contact form, and want to offer option to also be added to the newsletter list, (which would then go to Mail Chimp, for example...)
How to include a yes I want to, or No I don't... and then the appropriate action?

Our current integration feature with MailChimp allows you to offer to your users whether they want to be added to your mailing list or not.
However, I think your question has to do with making conditionals logics to either go to a form with an integrated newsletter service or not. On this case , the only way would be to make 2 forms. One that would contain the question and If the person answers yes, then redirect to a custom Thank You URL (which would be the second form already integrated with the newsletter service)

I do want to know if they want to be on the Mail Chimp newsletter list or not…
And as an additional question on the bottom of a form that already contains other questions and info we might be collecting…
So somewhere on or at the end of the form… for instance…we might say:
May we include you in our newsletter list?
Yes… (then, goes into Mail Chimp)
No… (does not go into Mail Chimp)
I don’t see how to do that on a Jot Form.

You will need to open you Mailchimp integration and go to the Choose a List stage. Then, select the Send If Choise is checked option for Only Send Drop Down and click on the Update icon. Once you click on that button, confirmation Check Box (with one option) will be added to you form.
Sorry for a little non intuitive interface on that action.
