My account has been auto suspended. What did I do wrong and how can I recover it?

Your account was autosuspended because in one of your forms there is a field asking for a password (Senha usuário).
http://www.jotformz.com/form/42963578705669I have checked the form and I noticed that the field is used just to show extra fields in the form and it is not a password for an account.
Your account has been activated and you should be able to use JotForm again.
