    Hi,

    I understand that you want to have a smart form that supports conditions. This is supported in JotForm. You can do this by following the guide at https://www.jotform.com/help/57-Smart-Forms-Using-Conditional-Logic.

    With conditional logic, you can hide/show different fields according to answers. For example if you want to show field 3 and 4, when the answer for the question is 2, you can build a condition like this;

    Hope this helps, have a good day

    Thank you for clarifying your concern.

    I believe what you are looking for is the Appointment Slots widget.

    Since you want to prevent an option to be selected again if it was previously selected by another respondent, then you need to set the number inside the bracket to 1 when setting up the available options on the widget settings.

    If an item was already selected, it will look like this to the next respondents.

    I hope this helps. If you have other questions or concern, please do not hesitate to let us know.
