How to insert a section in my JotForm?

Hello,

I need to insert a section just under the "phone number" section and just above the "type of inspection," section.  

It would be a simple section asking for Realtor name:     Phone: Email:

I tried but I couldnt do it.

You cannot insert a field into the "Main" form as the payment options where created separately and embedded as iframe. You'll need to edit the "PayPal" and "Check" form to insert fields.

Hello and thank you for your help.  

I made the changes to the form on the PayPal and Check form fields  

After copying the link to the website (homeinspectidaho.com), It now comes up (when I click Order Inspection) without the choice of "Check" or "Credit Card" at the beginning of the form. 

What do I need to do to fix it?

I have checked the website and I see what you mean when I clicked the ORDER INSPECTION button. Upon inspecting I noticed that you have copied the URL of the ORDER INSPECTION - Check form in your account to your website rather than the ORDER INSPECTION - Main form's URL — https://www.jotform.us/form/81774327586166.
