Account suspended

Hello,

My account has been suspended. Could you please say as to why it was suspended and how we can go about reactivating it. 

 Upon review of your account I found forms https://www.jotform.com/53004453803143 , https://www.jotform.com/53004523350138 ,   https://www.jotform.com/53004913116141 and https://www.jotform.com/53144377068156   were requesting  Social Security and Driver’s License Number.

Please be aware that this is not allowed  because it's considered a phishing activity, which violates our Terms of Use 

But it is allowed with the two mandatory widgets listed below:
