Account Suspended

Our account has been suspended.  Please advice as to why?

Upon review of your account I found form https://www.jotform.com/80307859784167 is requesting Social Security Number.

I recommend to check the Terms and Conditions. 

In regards to the collection of Social Security Number it is allowed with the mandatory agreements and signature widgets. 

Here are some widgets you can add to the form (signature fields):

You can also add this Terms of use widget (for the agreement), if needed,

Your account has been reactivated and the form has been added with the mandatory widgets, you can modify them to fit your bussiness.

Thank you for your reply and explanation. Your form begins with hpps and I thought that was the encryption needed?

Please advise what I would need to do.
