How can I change my username and password?

We can change the username for you.

Please provide at least 3 unique username so that we can check for its availability.

Please take note that period "." and spaces are not allowed anymore.

Note: If you are using custom URL for your form, it will no longer work since the username got changed. You will need to change the URL to have the new username in it instead.

For your password, you can change it in your profile settings>password then click edit.
