Can you help in creating a fillable PDF form?

PA VFW AUXILIARY used Jot Form for reporting community service hours.  It's fill-able and sends an email directly to the chairperson. I need to do something along those lines, but, not quite as involved.  Could I get help doing something like that?

     Let me start by saying I'm not very computer savvy.    I'm attaching a word document to give you an idea what we're looking for.  Yes, we'd be looking for a Fillable PDF.  We would want it to send notifications to my email address.  And, if possible, have it insert totals automatically at the bottom.

I'm not sure if you can attach a word document to the forms, however, you can send us a screenshot of the document.

Do you have a link to the form of PA VFW AUXILIARY ,so that we can have an idea on what you want your form to look like?

     Well, I'm such an airhead that I even forgot to attach it.  (big sigh)  Just in case you can work with it.    Community Service Report Form (PREFERRED)This is what we click on to file our VFW Aux. reports.  Hope that works.

okay ~ that's probably a lot more in depth than what we'd need. We don't even need a place for names.  It would all end being filed by Region and Chapter number.  Let's see if I can give you a better idea.

  This is pretty much what I've been working with in the past.  But, hoping we would get better response if they could just 'fill in the blanks' and hit send.

To clarify, are your trying to create a report? This seems to be a report. You can however, create forms to get the information, Reg.Chapter, date ,active, donations and totals.

Then create a report for the submission.
