Please assist asap, I keep getting a message after I complete a jot form I created of "Only One Entry Allowed" I am unsure what is going on because I have even deleted the persons info on a separate jot form thinking that was the cause but it did not help,, please assist asap, I have a hipaa compliant setup and these jot forms must work properly or else my credibility and business is in jeopardy

This is happening because the Unique Question is enabled. This feature prevents duplicate or multiple submissions using the same answer.
Please turn this off in the Form Settings, here's how:
1. In the Form Builder, click "Settings" in the top menu. In the "Form Settings" click the "Show More Options" button.
2. Scroll down and look for "Unique Question". After that, change it to "No Check".
