Why Is My Account Suspended?

Hi, my business is really affected by this.  Why is my account suspended?  The information I request on my forms is "by permission" from my clients.

One form is asking dream travel related questions (i.e. what's your budget? how many people do you plan to travel with?  what are their ages?  where do you want to go? etc.).

One form is asking specific questions about a particular trip (i.e. what are your dates?  how many in your party?  ages?  access restrictions?  meal restrictions?  etc.)

One form is a credit card authorization for a group cruise this December.  Every deferred payment REQUIRES a different authorization, regardless if it's the same card number.

As you can see these forms are travel specific for me to do my job.  Please release my account from "jotform jail" as soon as possible.

Phishing. You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

If you want to have a payment field inside of a form, you have to use one of the JotForm payment tools.

You are also collecting Driver's License. This can also be against our Terms of use. If you want to collect this type of information you have to add a sort agreement and an e-signature field to the form for users to confirm/allow/agree that you are collecting the information for a purpose.
