Your forms are originally tagged as phishing due to the password field. That is why our form reviewer team has suspended your account. However, looking further into your forms, I see the password field is used conditionally. 

I have reactivated your account, but I would suggest renaming the field not to mention the term Password. Use something, Enter Authentication.

I have already reactivated your account. Please check and follow my recommendation to change the term "password" to "Enter Authentication", of course using your current language.
