How to create multiple rows in a configurable list?

    I'm wanting to create a list with 4 rows, each row containing one checkbox on column 1 and 1 dropdown box (with different values) in column 2. Please advise.

    If I understand your question correctly, you want to display 4 rows in configurable list widget but the checkbox & dropdown value in each rows needs to be different. Is that correct?

    Unfortunately it is currently not possible to show different options in checkbox/dropbox question of each row. Please note that configurable list widget will only duplicate the rows and it is not possible to change the option value in different rows.

    I would suggest you to please add multiple configurable list widget in your form and inject custom css code in three widgets to hide the header text.
