How can we have the field description show up on submission?

For example, on my form we require our borrowers to read the disclosure before signing. However once the information is transferred to my dropbox, the disclosure is not there and only the signature shows up on the form. 

Is there another way to include this information. 

I have included a link to the finished application along with a screenshot of the disclosure. The disclosure is of the utmost importance here.

The description text is not included in the submission details. As a workaround, you may consider adding a Short Scrollable Terms widget field to the form.

The terms content will be included in the submission details and PDF.
