Why is my account say suspended when I try to login

    Unfortunately, your account was suspended because one of your forms violated our Terms of Use. We examined your form and noticed that it was obtaining sensitive information in an insecure way (in this case, social security number). You can see a fragment of the form that violated our Terms of Use in the image below:

    As our Terms of Use states:

    "You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

    I am going to subscribe to the $39 plan with HIPPA Compliance. How can I
    make the social security number secure on the form. We need to get that
    information on this form.
    I will purchase the new plan when I get to the office tomorrow.

    As mentioned in our terms of usage as shown below, you may collect SSN numbers using your JotForm. However, it is required to add a Terms & Conditions widget with your organization's terms and a Signature field so that the user can accept it to provide the necessary information using your JotForm.

    Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information
