Where am I supposed to place my unique 11 digit JotForm ID and secure server URL so that my form is secure? Your support page says to append the number to the end of this url- https://secure.jotform.com/form/. ... where do I do this?

To make your form secure just change it from http://form.jotform.com/form/12345678912 to https://secure.jotform.com/form/12345678912 - simple as that!

But my question is WHERE do I insert that secure URL?

Since that's a URL, just place it in your browser's address bar and hit [Enter]. If you wish to embed the form in a web page, the page itself must also be secure. Please see How can I receive SSL Submissions for more information.
