Can you import our document?

    Please if you could import  the only document we have on file.

    I checked your account (newageballers) and I did not find any forms on it. Are you referring to a different account? 

    If you are planning to import a PDF, Excel or Word form into JotForm, then I'm afraid that is not possible. We do not have a tool that can convert those forms into an online web form.
