Upgraded our Jotform account, but forms still have Jotform branding on them

Hello, we upgraded our Jotform account, but the embedded form on our website https://legacytree.com/checklist, still has Jotform branding on it. What do we need to do to have this removed?

You may disable the branding on your account settings page, please go here and un-check the JotForm branding option: https://www.jotform.com/myaccount/settings
