Have to upload received form data as testimonial page?

 Here is what I want to do

I want to create a form that I can email to a client, have them fill it out, send it back to me where I can upload it to my website testimonial page with the stars. I don’t want a testimonial form

page and a testimonial page at the same time on my website. Can you help me with that, yes or no?

You can create a form and email its link to your clients. When they fill it, you will receive submission to your email. 

I'm not sure how testimonial page works, but you could use PDF version of submissions received upload it, if that can work.
