Account suspended?

    Enquiring as to why my account is suspended?

    Your account was suspended because of this form: "Employee Information" and the reason is that you're collecting IDs (personal ID, driving license, passport etc.) which is considered as sensitive information.

    We need an explanation from you why are you collecting such info and for what purpose.

    We collect this info from new employees as most companies do. I work remotely and am never face to face with our staff so using JotForm is a perfect way to get all our payroll info from staff.


    I am happy to amend my form to comply with any T&C

    As per your request, we have now deleted the ID upload field from the form and activated your account. You may now login to your account and manage the forms normally. Please refrain from adding such fields on any of your forms to avoid the suspension of the account.
