I have checked your form and it seems your email notification has no fields in it.
May I suggest deleting this notification email and adding a new one.
By adding a new email notification, it would get all the available fields in your form and add it to your email content.
Please give it a try and let us know how it goes.

Usually you don't need to add your email address in the email recipient as the form builder will automatically add the email address in your account once you have added the new email notification.
You can however, double check it to be sure.
Your email address should be below your notification.

I have added a new notification email in your form. You can delete the previous email notification without the email content.
