I recently discovered that I am no longer receiving form submissions, nor is the site recording new submissions in my inbox.

I checked your form and there are 9 submissions there:
I don't see your submissions in email logs, so I'm not sure if you did any change in your Notifiers?
Also, I did a test submission in your form, please confirm if you received it?
Please advise.

However, I can not find any test submission, which you have mentioned. The latest form submission is from 2018-05-27. Have you deleted your test submissions?

I have attached the email I received from your latest test. Why does it
show the contact details repeatedly? I did not set up the form to
repeatedly request contact info, and it never did that before.

Apologies, I have filled out all the fields on your form. It appears that you have several pages requesting Name, Email and other info. The pages are skipped based on users' answers. Since I have filled out all fields, you have received all my answers via email.
Also, I have noticed that the Next button is named as Submit on your form. Please make sure that when testing you have also clicked the final Submit button to see the Thank You page and submit a form:

answers. But it should not repeatedly ask for contact info. Furthermore,
the form previously only showed 1 contact info page depending on what the
user's answer was. The user would then click "submit" and see the Thank you
page.
When I click "submit", I see the contact info request again, not the thank
you screen.

Please note that Next button can be used to navigate the form to the next page or other page based on the conditions set. In order to submit the form, it is required to click on the Submit button the form. The next button cannot act as the Submit button. You may place a Submit button on the first contact information and show/hide it based on the Gross income or as per your requirement.

If I am understanding correctly, it sounds like you're saying I have
renamed the "Next" button to show as "Submit," however if I do not want to
show all the pages, I need to use a specified "Submit" button. I wonder
why it always worked in the past. That said, in order to use the "submit"
button I needed to eliminate the "back" and "next" buttons. It took me
for-ev-er trying to figure out how to get a page break without the buttons
to discover that I could hide the buttons. Thank you for pointing me in the
direction I needed to go.

Is the issue resolved?
You do not have to remove the Page Break to your form.
You just need to correct the "Next" button and rename it. Here's a screencast sample:
If you want we can edit your form and rename the correct "Next" button.
