Unable to locate form

I created a form, https://form.jotform.com/81545007765156

and I had someone complete the form and submit some documents to it, but when I go to find it in Jotform, it does not exist. 

Where is it?

I can see this form has been created by the user "" hence you will need to login as that user. On MyForms select the form and click on View Submissions on the top. And that should display the data including the files.
