I would like to re-activate my suspended account. How will I get this done?

Hello Michael, these forms were found on your account as suspicious of phishing activity because you're collecting credit card details, social security and driver's license numbers and copies of driver's license/IDs.

Gathering these types of data violates our Terms of Use.
As a gentle reminder, card numbers, expiration dates, security codes and card holder names are considered critical and sensitive information. The only services allowed to collect such data are those that are DSS/PCI certified.
In line with this, you must take the following actions:
1. Integrate your form with one of our Payment Processors.
2. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Authorization Only". To learn more, kindly follow this guide on How to Enable Payment Authorization.
Also, if your purpose is not to collect social security and driver's license numbers from the general public, but from your own customers, then you need to comply with two things.
1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.
2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.
However, we do not allow upload of any sensitive personal information like passports, social security card, driver's license, credit/debit cards, bank statements and photo IDs.
I removed the offending fields on your forms and reactivated your account. Please refrain from re-adding these kinds of fields to prevent permanent account suspension.
