Why is my form not showing the question for the 2 configurable Lists?

I cannot see which question is submitting on the submission form.

Are you not able to see these 2 questions in the email after submission or the submissions received?

In Email notification content please add these 2 question's text, then you would be able to view the questions on the notification received after submission.

In submission page, I am able to view these question.
