My login is supsended!

    Hello

    I am trying to login as I just sent out an important email to all vendors for an event of ours...and a vendor brought it to my attention. Why would this have happened? How can we remedy this. I sent out this email with the attachment form to over 100 vendors. PLEASE HELP ASAP!

    Upon review of your account I found form: https://www.jotform.com/80925098284163 was requesting for credit card information in plain text fields.

    Please be aware that this is not allowed  because it's considered a phishing activity, which violates our Terms of Use
