Why has my jotform stopped showing up on our website page.

    We created a form for potential customers to fill out and it has worked pefectily until the 28th of November.  Now when people vistit our website (hosted by imcreator.com) the page is blank.  When I logged into my site through imcreator, the jotform page comes up with this: v8_build_source.js:49: ReferenceError: V8Config is not defined load ( V8Config.CACHEPATH + form_id + '.js' ); ^

    I don't know what triggered this and when I attempt to copy and paste the page code as I did originally nothing comes up but the same blank page our customers are seeing.  Can anyone help?

    I have check your webpage source and I can see some extra scripts, I suggest you to delete it;

    I also notice the bottom part of the webpage, it seems like being cut off. Please check.
