Filtered Content - Unblock form addresses

To whom it may concern,

We are using jotform on a computer that we are filtering content on. I have allowed jotform.com, forms.jotform.com, submit.jotform.com and the unique URL for each of our forms. The forms are all accessible except for a specific section that accesses spreadsheet information via one of your widgets. That particular section of the form comes up blocked. I am wondering what I need to unblock to allow access to that specific part of the form. 

I have included the URL to the form we are having trouble with below. The specific section that is being blocked is called "Employee ID (Last 6 of SS)". I have also provided a screen shot. 

Please also whitelist the following addresses:

jotform.io
widgets.jotform.io
data-widgets.jotform.io
