Info deleted from my forms

the addresses are deleted from my form or not showing. I check the edit form option and it had "address will be hidden" however I right clicked and selected show, however the addresses are still not showing.

From the screenshot you provided, I was able to identify the form you are having issues with.

I checked the form's submissions and I see that most, if not all, data are missing for the address field.

It looks to me that the original field (address) was deleted and another address field was added. If that is the case, then I suggest that you try reverting to the time when the original address field is still in your form using the Revision History Tool on the builder page — How-to-View-Form-Revision-History

Deleting a field will also delete the submission data associated with it.
