Unable to locate one of my forms

    I logged in and found one of my forms in the Trash that I wanted to use.  I chose to restore it and now I don't see it anywhere.   It was a drivers training form.   Can you help?

    In My Forms page, you can use the Search box to sort the forms. Here's a screenshot:
