Form won't work on website

    I just went in and changed parts of my form on http://htchatters.org and the form will not show up when the link is clicked. Please help me with this ASAP!

    Please confirm if you are still having an issue with your form on your site? I checked your site and can click on the button that redirects to your form without issue.
