How do I print the information in the form along with a signature?

    Greetings! We are using JotForm to obtain signatures from parents to authorize treatment by a camp nurse or by emergency personnel and doctors in the event of a medical emergency. The problem we've run into is that the survey response shows only the parents' signatures, not the information about what they have agreed to. There was one case this year where an emergency occurred requiring the camper to be treated by a physician at a hospital. We had a record of the parent's signature authorizing this, but no way to print out a copy of the form along with the signature to show to the doctor. How can we solve this problem? Thanks!

    I believe that you are referring to the form https://www.jotform.us/form/72336407161148 in your account. Do you mean to display the Authorization text in the email or the submission?

    If so, you may add the text to the email body by copying and pasting it. If you are trying to print the submission from the PDF report or submission, you may add the fields to the submission by following the guide below:
