What is the best form element for an option where someone can check an answer and also give further information?

I'm looking for an option where someone can check an answer and also give further information.

Example:  Have you ever had a head/neck injury?  <>No <>Yes (explain): _________

What would you guys recommend for this type of question?

You may add a radio button for the question and a Long text entry field for the explanation.

You may show/hide the explanation field based on the selection of radio field by setting up a condition.
