Request to cancel subscription and refund the payment.

    I canceled my account earlier this week since the event I am using the form for has come to a close, BUT I just received a receipt for a new month payment. 

    Please return this money to my account and close my account completely on your end.  Since it is a deleted account, I can not log on to do anything else to end these payments myself.

    I'm interested in using your service again in the future, but if this is not resolved soon, I will research another form site that can support my organization's needs as well as take a phone call to resolve simple issues like these.

    The number you have on this receipt isn't even legitimate.  That's a scary sign.
