How to Embed PDF into Form / Have it submitted to my email

    I want to embed an external PDF document into one of my forms so that a user can fill out this PDF form and when they click submit, the entered information will be sent to me. Is there a way to do this? For example, if I wanted to embed a W7 PDF Tax form.

    Hi Thanks for the reply...I looked at this method and tried it out. It seems that the user can only view the uploaded PDF with this method, is that correct? I would like to upload a PDF that users can fill out / type information into as well. Is there any method to do this? Or to replicate an existing PDF so that users can fill in information?

    It's not possible to attach a PDF that can be filled by user. However, you can convert your form into PDF format. User would be able to fill and submit using your fillable PDF form.
