To my understanding, you have purchased an item through a form made on JotForm, is that correct?
If that is the case, I suggest that you try to contact the form owner.
I believe you have received an email when you submitted the form. Usually, it has a "Reply-To" email address. You may use that to contact the form owner.

I'm sorry for the inconvenience, but you probably clicked the link to create a form from the email you received as a confirmation that you submitted a form owned by one of our users.
I understand that you submitted a Return Order Form, however, the one that you need to contact is the form owner. JotForm is an online form builder that was used by the owner of the Return Order Form that you submitted. Check the email that you received and verify the "Reply-To" email address to contact the form owner.
