My form is not visible

My New Client Information Form is not visible when I want to preview it. It is also not showing up form my link on my website. I can edit the form but I cant view it. Can you tell me how to fix it?

I was able to see the issue that you described when I created a clone version of your form. However, while trying to find the solution, I checked your form again and I believe the issue is now fixed.
