How to create a quiz form that emails the result or display it on thank you page?

Before I recreate the wheel, I thought I'd check to see if there is a template that lets users submit answers to a test and either emails the person with their results or displays the results on the thank you page.  Thanks for your help!

You can use Jotform's quiz app here https://quizform.jotform.io/. This will allow you to create your own quiz form and sends the result to your preferred email.
